OntarioMD’s Health Report Manager Named a Finalist for PwC Canada’s Vision to Reality Awards

HRM is a digital health solution that securely delivers patient reports (e.g., discharge summaries) from hospitals and specialty clinics seamlessly into the electronic medical records (EMRs) of nearly 9,000 clinicians. Reports appear in minutes; a vast improvement over the days or weeks it took for mailed or faxed reports to arrive - reports are not lost or misdirected. 

Information about a patient's hospital visit now reach their family physician before they do. Receiving reports faster has meant that clinicians in the community have the most up-to-date information they need to be proactive and follow up with patients. Care can be better-coordinated and the management of potential complications and hospital readmissions reduced, especially for patients with complex conditions. These outcomes of using HRM are aligned with Ontario’s Patients First Digital Health Strategy which focuses on patient-centred care and improving the patient experience of the health care system.

HRM reduces the administrative, paper burden on physician practices, and saves them approximately 30 minutes per clinician per day by not having to handle paper reports. Each month HRM delivers 1.25 million reports to Ontario clinicians, and this number increases every month. 

HRM includes eNotifications that inform clinicians when patients are discharged from an emergency department or in-patient unit in real time, and before the discharge summaries arrive. “My practice receives eNotifications and it's been a positive experience. I followed up with a patient shortly after he was discharged and he was blown away that I was able to follow-up with him so quickly," said Dr. David Kaplan.
