ConnectiCare Utilizes Mobile-Enabled Platform to Increase the Reach and Impact of Care Management Services


BOSTON, MA--(Marketwired - October 07, 2015) - According to the Centers for Disease Control and Prevention, nearly 50% of people say they are not doing the right things to manage conditions like diabetes. ConnectiCare Inc. and Affiliates, a leading Connecticut-based health plan, will utilize Boston-based Wellframe's Intelligent Care Management Platform to empower individuals to take control over their chronic health conditions by delivering a daily, customized care plan directly to members through an application on their mobile phones.

This customized care plan translates complicated medical instructions into an easy-to-follow, daily health checklist, which both the member and the Care Manager use to track progress. In addition, ConnectiCare members will be able to use Wellframe's mobile application to securely message their ConnectiCare Care Manager. By doing this, ConnectiCare will be extending the reach of their Care Management team, while expanding the health management services provided to members.

With other health systems, Wellframe's program has historically engaged more than 80% of its participants, for nine or more months. Additionally, health plan Care Managers using Wellframe, have reported that they are able to reach more patients and have more meaningful interactions.
