Longitude Health Approved for NIH-NIDDK Study of Collaborative Care Support(SM)

$210,000 to Study How Patients With Type 2 Diabetes and Multiple Comorbidities and Their Primary Care Physicians Use the Web-Based Behavior Change Program

SOMERSET, NJ--(Marketwire - April 20, 2010) -  Longitude Health, Inc. (www.longitudehealth.com), provider of web-based, guided health and wellness solutions, today announced that its Phase 1 Small Business Innovation Research (SBIR) grant to support the feasibility assessment of Collaborative Care Support(SM), the company's proprietary system for integrating primary care physicians into patient-centered, multiple-risk factor intervention for chronic condition management, has been funded. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) of the National Institute of Health (NIH) awarded this approximately $210,000 grant titled "Feasibility of a Web-based Collaborative Care Support(SM) Model to Improve Care for Patients with Type 2 Diabetes and Cardiovascular Comorbidities."

The phase 1 research will entail delivery of services to a panel of persons with diabetes and other cardiovascular comorbidities while also engaging with their primary care physicians in the behavioral change model.

Persons with diabetes often have complex related health problems and are often underserved by traditional disease management programs. Collaborative Care Support(SM) provides a continuous link between patients, their physicians and Longitude HealthGuides (coaches) as they create, manage and track custom health improvement plans that address the full range of health challenges and opportunities faced by each individual. This model of self-management support may fill a niche in the Patient-Centered Medical Home model. Physicians benefit from better time management, workflow integration and compensation for their role in care oversight.
