Telecare Partners With Sacramento County to Help Residents With Persistent and Serious Mental Illness Achieve Greater Independence

Sacramento Outreach Adult Recovery (SOAR) Aims to Help Clients Avoid Hospitalization

SACRAMENTO, CA--(Marketwire - February 9, 2010) - Telecare Corporation has opened its first Sacramento County-based program, Sacramento Outreach Adult Recovery (SOAR), to serve the needs of individuals with persistent, serious mental illness who have the ability to live more independently and avoid long-term hospitalization.

Using the research-proven Assertive Community Treatment (ACT) model, SOAR is a nationally-accredited program that will provide intensive support services for up to 100 clients, 18 years of age or older. SOAR clients are individuals who have previously had long, expensive stays in a locked state Institution for Mental Disease (IMD) -- or face that possibility -- but who are ready to begin the transition to community-based care.

"Our clients will receive the support and resources needed to help them learn to make personal choices and gain the skills needed to live more independently," said Dennis Dow, MFT, Program Administrator. "Telecare's recovery model is a client-centered, individualized approach emphasizing personal choice and empowerment."

Referred to SOAR by the Sacramento County Division of Mental Health Services, individuals selected for SOAR do not require a secure environment, but require additional support to make the transition to independent living.

"SOAR services fill a gap by providing intensive services and strong member-staff relationships that enable members to transition to and remain in the community successfully," said Clinical Director Anne Robin, MFT.

"Among the benefits associated with ACT programs are a reduction in the use of inpatient hospital bed days and a decrease in the number of visits to local area hospital emergency departments," said Stacy Starr, Program Coordinator, Adult Mental Health Services, County of Sacramento. "Telecare has an outstanding national reputation for its programs and recovery-model and we look forward to the positive impact their services will have on hundreds of lives."

SOAR services include:

-- Clinical risk assessments
-- Medication management and training
-- Individual service planning
-- Psychiatric services and individual therapy
-- Psychosocial rehabilitation and social skills training
-- Symptom education
-- 24-hour crisis care services for members and families

housing environments, and support in daily living
