Web-Based Care Management Solution Embeds Member Health Decision Support

Health Plans Can Now Leverage Healthwise Decision Support From Within CareRadius

CHICO, CA and BOISE, ID--(Marketwire - April 6, 2010) -  Landacorp and Healthwise announced today an agreement to integrate the Healthwise® Knowledgebase into Landacorp's CareRadius suite. This integration will improve the ability of health plans to access consumer health content and decision support tools and will ensure consistent delivery of clinically validated information within the care community.

The CareRadius suite, a Web-based care management solution, brings together the data and activities related to a member's health into a single, member-centric view. Health plans using CareRadius will have access to more than 6,000 Knowledgebase topics on chronic conditions, wellness, medical tests, procedures, and medicines. The depth and breadth of information in the Knowledgebase will support care managers in educating members about their medications, symptoms, wellness plans, and routine screening tests.

Here is one example of how this integration can help improve health outcomes and reduce cost:

CareRadius uses automatic triggers that comply with HEDIS measures to trigger alerts for the care manager when it is time for a member to have a screening test. Care managers can now use the Healthwise Knowledgebase, including the decision aids, to educate members on the screening tests most appropriate for maintaining or improving their health status. The care manager can send educational content to the member to reinforce the information learned during their call.
