AlayaCare and Its Partners Awarded OCE's AdvancingHealth Innovation Fund

Collaboration to Launch Innovative Model of Care for Home and Community Care Patients

TORONTO, ON--(Marketwired - November 30, 2015) - AlayaCare, providers of next generation home care software solutions, announced today that it has been selected as an award recipient of the AdvancingHealth Program. Along with its partners, AlayaCare will launch virtual care technology that enables home care providers to administer care via remote patient monitoring (RPM), telehealth and video conferencing. The AdvancingHealth Program, managed by Ontario Centres of Excellence (OCE), is designed to facilitate partnerships between Ontario public healthcare organizations, companies and academic institutions to increase innovation in Ontario's public healthcare sector.

AlayaCare, along with Southlake Regional Health Center (Southlake), CBI Health Group, Professor Carolyn McGregor at the University of Ontario Institute of Technology (UOIT), and the Centre for Excellence in Economic Analysis will seek to reduce hospital admissions and emergency department visits for chronically ill patients. They are one of five teams awarded up to $250,000 to improve patient outcomes and enhance patient experiences through the use of innovative technologies.

AlayaCare's role within the project is to provide innovative technologies such as a Patient App, Telehealth Dashboard, Family Portal, RPM software, and Machine Learning algorithm-based clinical support tools, which allow for better coordination and quality of care across the healthcare continuum.

The key deliverables of the project impacting the Ontario healthcare system are:

  • Reduced hospital re-admissions and Emergency Department (ED) visits
  • Positive health economics (i.e. reduced health system utilization)
  • Improved patient satisfaction
  • Positive experience for the family health teams at all hospital partners as evidenced by a desire to see the program continued and expanded
