Senior Care supports medication therapy management (MTM) consultations that focus on optimizing seniors' use of medication and identifying potential health problems common in the aging population.

Seniors, the fastest growing segment of population in the U.S., are the nation's largest consumers of prescription and over-the-counter medications. They take twice as many medications as do younger adults. On average, older adults take 4-5 prescription medications and 2 over-the-counter medications a day. Seniors with chronic diseases and some acute conditions reap the most benefits from proper use of appropriate medications. Unfortunately, the complexity of the regimen can present high risks for these patients, who are often challenged to follow the prescribed therapy properly. Non-adherence accounts for more than 10% of older adult admissions to hospitals, and nearly 25% of nursing home admissions.

With Senior Care pharmacists conduct comprehensive senior assessments that allow them to screen for lifestyle concerns and medication appropriateness providing an opportunity for pharmacists to teach patients about their medications and create customized medication services for safe and effective drug therapy.
