Collaboration Creates a Unique Model for Sharing Data Between Health Information Exchanges to Improve Patient Health and Safety and Reduce the Cost of Care

FORT WORTH, TX--(Marketwired - Aug 8, 2013) - Sandlot Solutions, a healthcare information technology company, and Baylor Quality Alliance (BQA), an accountable care organization (ACO) affiliated with the Baylor Health Care System, today announce a partnership that will support the management of patient information across multiple healthcare providers and facilities. This health information exchange (HIE)-to-HIE relationship establishes a unique, patient-centric model for data sharing and represents an innovative approach toward care coordination that will improve patient safety and outcomes, while reducing costs associated with delays in care and duplication of services.

This level of health data exchange supports not only the improved health of individuals, but it also provides a secure platform for achieving true population health management.
