CHC Receives $1.2 Million in Federal Grant Commitment for Nurse Care Coordination Program

MIDDLETOWN, CT--(Marketwired - July 15, 2015) - Community Health Center, Inc. (CHC), Connecticut's largest primary care provider for the underserved population, headquartered in Middletown, CT, announced today that it has received a federal grant from the Health Resources and Services Administration (HRSA) to improve patient's health outcomes by expanding nurse care coordination to its state-wide network of health centers over the next three years.

The project has three primary goals: to improve and expand the Interprofessional Collaborative Practice environment agency-wide by developing CHC's care coordination program, to build RN leadership skills and experience; and to build RN student competencies in Interprofessional Collaborative Practice.

"This initiative continues the investment CHC has made to ensure that front line teams receive the training and development necessary to enhance the patient-nurse relationship. It will redesign the role of the primary care nurse from its origin in the academic setting by ensuring a diverse and innovative curricula paired with high quality clinical experience, to enhancing the on-the-job support and training to intervene at the most critical time for our most complex patients," explained Mary Blankson, DNP, APRN, FNP-C, Chief Nursing Officer for CHC.

This project will be strengthened by collaborations with Quinnipiac University and Middlesex Hospital Homecare, and will also facilitate interprofessional education opportunities with nursing, pharmacy and medical students.
