CHRISTUS Health Partners With Wellcentive for Large-Scale Population Health Management Deployment

End-to-End PHM Solution to Enable International Health System to Collaborate and Manage Risk Across the Care Continuum

ATLANTA, GA--(Marketwired - Feb 11, 2014) -  CHRISTUS Health, an international, Catholic, not-for-profit health system, has selected Wellcentive's suite of products and services to empower providers to systematically manage the health of their patient populations. Wellcentive's solution will play the key role in CHRISTUS Health's plan to take on the additional risk of managing patients in the primary care environment and collaborating across the continuum of care. The platform will enable the integrated delivery network to implement a consistent strategy for population health management that aligns physicians, hospitals and other practice sites. This large-scale technology deployment will help the organization aggregate data to establish and standardize best practices on how care management will be delivered to continuously improve clinical and financial outcomes.

CHRISTUS Health's enterprise population health management strategy depends principally on Wellcentive's ability to aggregate data from multiple systems and vendors. The Wellcentive platform will interface with the health system's MEDITECH hospital patient record system, its health information exchange infrastructure, various ambulatory electronic health records and other systems. This integration is especially critical in working with information systems entailing different reporting requirements across different regions. Wellcentive's application then translates that data into meaningful health information and program-specific reports that promote collaboration across the continuum of care and help practices optimize clinical and financial outcomes.

The first phase of the Wellcentive implementation will focus on managing Medicaid populations in the Corpus Christi, Texas region as part of a Delivery System Reform Incentive Payment (DSRIP) project. A growing number of Medicaid beneficiaries are entering the healthcare system, and managing their care effectively is more important than ever. Texas, where many CHRISTUS Health facilities are located, is one of several states to pilot a Section 1115 Research & Demonstration Project to improve Medicaid management and reward providers for improving the cost and quality of care.

Upon completion of the first phase, Wellcentive and CHRISTUS Health will execute against an aggressive deployment schedule to implement the platform across the remaining chosen sites.
