State of Michigan Duals Demonstration Medicare-Medicaid Plan Selects ConcertoHealth; Four Care Centers Add 200 New Jobs, Infrastructure to Greater Detroit Area


IRVINE, CA and DETROIT, MI--(Marketwired - Oct 26, 2015) - ConcertoHealth (formerly Fidelis SeniorCare) today announced it has expanded its footprint in Michigan by opening three additional Care Centers in the greater Detroit Area within 30 days, bringing improved access to healthcare for the complex medical Medicare/Medicaid patient population.

Patients with complex medical needs or dual-eligible status, represent about 20 percent of the U.S. Medicare population and in Michigan total more than 290,000 people.i,ii ConcertoHealth has more than a decade of experience caring for the Medicare, Medicaid, frail and complex medical needs patient populations. It was the success of ConcertoHealth's model that led to the company's selection by the State of Michigan Duals Demonstration Medicare-Medicaid Plan program.

ConcertoHealth integrates clinical care with social services to ensure patients and their families receive the individualized support they need to improve health outcomes. In addition to providing better access to care, ConcertoHealth's new Care Centers have created an infusion of 200 new jobs and infrastructure to the Detroit area.

  • Care Coordination for each patient available 24-hours, 7-days a week, including in-home care for non-ambulatory patients, bedside patient management in-hospital or post-acute settings, and management of transitions between care settings;
  • Chronic Disease Management and comprehensive preventive screenings, access to non-emergent transportation, as well as prescription assistance, the absence of which are key barriers to healthcare services.
