ConcertoHealth, Meridian Health Plan of Michigan Announce Strategic Partnership

Partnership Will Provide Meridian's Medicare & Medicaid Members Access to ConcertoHealth's Comprehensive Primary Care Platform

IRVINE, CA--(Marketwired - Jun 21, 2016) - ConcertoHealth and Meridian Health Plan of Michigan announced today a strategic partnership to serve Medicare and Medicaid-eligible patients in the Greater Detroit area. The healthcare companies have entered into an outcomes-based contract to improve healthcare quality and access for patients beginning July 1, 2016.

ConcertoHealth has extensive experience caring for Medicare, Medicaid and complex medical needs patients. The company integrates clinical care with social services to ensure patients receive individualized primary and preventive care, across and between settings -- from the physician's office, to the nursing facility or hospital, to the patient's home.

ConcertoHealth will serve members in Meridian's Medicare Advantage HMO plan, Dual Special Needs Plan, Medicare-Medicaid Plan, and standalone Medicaid plan (for aged, blind, and disabled recipients 55 and older).
