IRVINE, CA--(Marketwired - Dec 2, 2015) - ConcertoHealth today announced the appointment of Dr. Sundara Samavedi as its Washington State Medical Director.

Dr. Samavedi will lead the company's medical initiatives in the wake of ConcertoHealth's recently announced expanded partnership with Molina Healthcare in Washington. "He is considered a thought leader in the areas of adult primary care, chronic conditions and geriatric care. We are privileged to have someone with his depth of experience and knowledge to guide our medical initiatives as we continue to expand throughout the greater northwest."

ConcertoHealth has more than a decade of experience caring for the Medicare, Medicaid, frail and complex medical needs patients. By merging social work with care management, ConcertoHealth ensures patients and their families receive the individualized support they need to improve health outcomes. The company provides Care Centers for patients who can get to a clinic, in-home care for those who can't, and its field-based care teams provide for those in-hospital, in a nursing home, or any post-acute setting.

ConcertoHealth gained a deep understanding of health care delivery complexities and financial risk management of complex medical needs patient populations during its long tenure operating as both a Medicare Advantage health plan and as a care provider through medical centers.

"ConcertoHealth provides the type of personalized level of care necessary for a growing, aging, dual-eligible membership population," said Dr. Samavedi. "With more than a decade experience in providing care to the Medicare population, I'm pleased to continue in this new role as the company continues to expand and provide better access to care to the complex needs patients in Washington."
