CoreSource Introduces Unique Suite of Physician-Driven Total Population Management Products

LAKE FOREST, IL--(Marketwire - November 13, 2007) - CoreSource has introduced an integrated suite of total population management solutions called CoreCare(SM) to the self-funded marketplace. The new services are designed to minimize an employer's medical plan costs by maximizing the health potential of every plan member via improved prevention, identification and management of healthcare risks within their employee population.

CoreCare is based on the philosophy that the best approach to health management comes from reinforcing the physician-patient relationship and providing the physician with the information he or she needs to help the patient. CoreCare combines the expertise of CoreSource's traditional healthcare and disease management services with the industry-leading data mining and medical analytics capabilities of Waltham, Mass.-based D2Hawkeye.

CoreCare(SM) Focus expands traditional disease and case management services to include plan members who are at a high risk of incurring major medical costs in the future, regardless of the costs they're incurring today. CoreCare(SM) Review integrates utilization management and case management to help ensure every day a patient spends in a hospital is appropriate and medically necessary.
