Deaconess Health System Partners With Evolent Health to Strengthen Its Value-Based Approach to Care

WASHINGTON, DC--(Marketwired - April 16, 2015) - Evolent Health, the leader in helping progressive health care systems lead, build and own the path to value-based care, today announced its long-term partnership with Evansville, Indiana-based Deaconess Health System -- a premier provider of health care services to three states and 26 counties. Together, Evolent Health and Deaconess Health System will drive enhanced care quality and outcomes while simultaneously lowering costs for patients, employers and payers.

"We're thrilled to partner with Evolent in the effort to elevate our model of care for all populations we serve," said Dr. James Porter, Chief Medical Officer of Deaconess Health System. "Evolent brings the technology, expertise and proven approaches to achieve deeper integration across our hospitals, physicians and allied services."

"This partnership reflects our dedication to providing preeminent care throughout our tri-state community. Health care reform created an opportunity to rethink volume-oriented processes in a way that significantly reduces gaps in care and coordination. We were an early mover in making this shift, and chose Evolent to speed the pursuit of our long-term vision."

Under the partnership, Evolent will work directly with Deaconess' leadership and physician teams across multiple areas: to optimize population health and medical home infrastructure and the associated investment returns; to establish and manage a financially integrated regional network; to secure value-based payer partnerships through Evolent's Payer Value Alliance; and to augment existing electronic medical record technology toward seamless, team-based patient care.

"It was critical to identify a partner with which we could gain immediate momentum in not only setting up our physicians to lead, but in also installing the necessary resources to ensure their success," said Dr. Dan Edelman, Chief Medical Information Officer of Deaconess Health System. "Evolent provides the underlying technology to combine and use data across multiple sites and systems, and the care management expertise to support staff as they carry out these evidence-based work plans."

"Deaconess is pioneering a new model of care, not only for the system itself but for the large geographic region it serves," said Evolent Chief Executive Officer, Frank Williams. "The entire Evolent organization, and especially our on-the-ground in Evansville team members, are excited to support Deaconess' commitment to continuously enhancing the quality of health care it provides to the persons, families and communities it serves."
