Louisiana's Largest Clinically-Integrated Physician Network Selects Aerial™ Platform to Aggregate and Activate Data From Across Care Continuum to Improve Care, Enhance Collaboration and Reduce Costs

PHILADELPHIA, PA and METAIRIE, LA--(Marketwired - Feb 4, 2015) - Medecision, a leader in population health management solutions, and Louisiana-based Gulf South Quality Network, LLC, (GSQN), today announced they will partner to create a technological infrastructure for GSQN, one of the nation's largest and most advanced clinically-integrated networks and pharmacy. Together, GSQN and Medecision will build one of the country's largest and most sophisticated data warehouses, which will be powered by Medecision's Aerial™ scalable, Software-as-a-service platform and will integrate electronic medical records, health claims, consumer and other data to support the financial and operational requirements associated with value-based care.

GSQN joins the roster of leading healthcare provider-based organizations collaborating with Medecision to support the three key pillars of effective, profitable population health. Together, the partnership will enable GSQN to accomplish this through:

  • Activation of big data - Highly-connected, real-time data acquisition, management and warehousing; provides a 360-degree view of consumer health from systems across the care continuum, including consumer data, EMR data, claims data, laboratory, RX and other sources
  • Enhanced insights and workflow - Predictive analytics for effective risk identification, stratification and business modeling; tied to care coordination and management workflow
  • Greater engagement - Transparency tools utilizing the latest in real-time reporting; mobile applications and social networking expose information to engage physicians, hospitals, consumers and their caregivers at the point of service 

Moreover, the Aerial platform will help GSQN aggregate, analyze and activate data from across its network of 1,600 physicians, tens of thousands of patients and their care teams, which include some of the largest hospitals, health plans and complex chronic care populations in the state. This collaboration among all stakeholders in the network will enable GSQN to better identify and close care gaps across the care continuum, thereby improving population health, reducing costs and significantly improving transparency and availability of health information to improve the consumer experience.

This will create a fuller, longitudinal picture of the care continuum for GSQN and more accurate health profiles for GSQN's patients, which, when coupled with Aerial's advanced predictive analytics will enable GSQN's physicians to:

  • recommend the most effective treatment protocols for each patient's individual needs;
  • perform risk stratification to ensure medical budgets are allocated and spent appropriately across populations;
