Hillcrest HealthCare System Selects Phytel for Population Health Management

Northeast Oklahoma Provider Chooses Phytel's Automation and Analytic Tools to Build Infrastructure for Quality Programs, Accountable Care

DALLAS, TX--(Marketwire - Aug 29, 2012) - Phytel, the leader in automated, provider-led population health improvement, announced today that Hillcrest HealthCare System (HHS), a network of six hospitals and 49 clinics in northeast Oklahoma, has selected Phytel to help HHS' physician group execute its population health management strategy.

HHS will deploy Phytel Outreach™ to 100 primary care physicians -- nearly half of the 195 providers at Utica Park Clinic -- to ensure that their patients receive recommended preventive and chronic care. The health system then plans to implement Phytel Insight™, which uses a web-based registry and advanced analytics to measure the effectiveness of quality initiatives and generate actionable data to improve health outcomes across a population.

Hillcrest is involved in multiple quality improvement projects with both commercial health plans and Medicare. Sixty Utica Park Clinic physicians were recently accepted into Medicare's Comprehensive Primary Care Initiative (CPCI), which gives primary care physicians the resources they need to better coordinate care for Medicare patients.

"We'll use Phytel's solutions to help build the infrastructure around the CPCI program and other quality monitoring programs," commented Jeffrey Galles, D.O., chief medical officer of Utica Park Clinic. "We can also use these tools to build additional infrastructure if we elect to move forward with an accountable care organization. Phytel's technology provides an ideal fit for us now and into the future."

Steve Schelhammer, CEO of Phytel, said, "Phytel is proud to partner with Hillcrest HealthCare System, which is embarking on a bold initiative to improve the quality of care for its patients. The combination of Phytel Outreach and Phytel Insight will give HHS' care teams the tools they need to manage population health cost-efficiently and at scale. Physician-led population health improvement is crucial to making healthcare more affordable, effective, and patient-centered."
