Kathleen Sebelius Joins Solera Health Advisory Board

21st Secretary of U.S. Health and Human Services to Help Chronic Disease Prevention and Management Integrator Scale Its Innovative Delivery Model to Meet Needs of At-Risk Americans

PHOENIX, AZ--(Marketwired - Sep 15, 2016) - Solera Health, an integrator of chronic disease prevention and management solutions, today announced the appointment of Kathleen Sebelius to its advisory board.

Solera Health is an integrated community health technology platform that connects patients, payers, and physicians with a new class of non-clinical healthcare providers, including community organizations, national partners and digital program providers safely and securely. In its role Solera contracts with health plans to deliver evidence-based chronic disease prevention and management programs, including the Center for Disease Control and Prevention (CDC) Diabetes Prevention Program (DPP), to its member base, to effectively empowering them to prevent and manage obesity-related chronic diseases, including diabetes and heart disease. 

Kathleen Sebelius is one of the foremost experts on national and global health issues. From 2009 -2014, Ms. Sebelius served in President Barack Obama's Cabinet as the Secretary of the Department of Health and Human Services (HHS), leading the effort to pass and implement significant health reform through the Affordable Care Act. In 2011, Forbes named Ms. Sebelius one of the 100 most powerful women in the world. Prior to serving as secretary of HHS, Ms. Sebelius was the Governor of Kansas, served two terms as the Kansas insurance commissioner, and four terms in the Kansas Legislature. Ms. Sebelius earned a master of public administration degree from the University of Kansas, and a B.A. from Trinity Washington University.
