Netsmart to Provide Care Coordination Technology for Care Transitions Network

Initiative in New York State Seeks to Reduce Re-Hospitalization Rates for People With Serious Mental Illness (SMI)

OVERLAND PARK, KS--(Marketwired - March 07, 2016) -   Netsmart will provide key care coordination and care management technology capabilities for the Care Transitions Network for People with Serious Mental Illness (SMI), a key initiative of the National Council for Behavioral Health. The announcement was made at the National Council Conference (NatCon) in Las Vegas, Nev.

The Care Transitions program in New York State will provide support for patients after their discharge from psychiatric hospitalizations, ensuring engagement with outpatient practices for coordinated services and care. The goal of the four-year project is to reduce all-cause re-hospitalization rates for people with serious mental illness in the State of New York by 50 percent.

Netsmart will provide care management and coordination solutions to support providers across New York State as part of the initiative. The Netsmart CareManager solution will support the workflow as individuals with SMI diagnoses transition out of the hospital back into the community.

According to the New York State Office of Mental Health, 82 percent of patients with hospital readmissions have mental health or substance use disorders. While these patients have psychiatric diagnoses, the majority (59 percent) of the readmissions are for medical disorders.

The Care Transitions Network will work with more than 4,000 clinicians across the State of New York over the next four years to implement pragmatic treatments for patients suffering from the most severe mental illnesses, including schizophrenia, bipolar disorder and chronic depression. It will employ a robust data infrastructure to ensure that primary and behavioral health care services are better aligned, inpatient and outpatient providers bridge the gap at discharge, and payer and provider priorities are better in sync.

The Care Transitions Network anticipates being able to avoid 1,150 hospitalizations and save $12.6 million in costs during the first year with proper medication use and care transition support. Over the four-year life of the project, 16,000 hospitalizations may be avoided for a projected $186 million savings.

The program is funded by a $29.4 million Transforming Clinical Practice Initiative grant awarded by the Centers for Medicare and Medicaid Services.

The Care Transitions Network is one of 29 Practice Transformation Networks around the country working to create new and replicable models of care for vulnerable populations and helping provider practices move into the world of value-based payments.
