DALLAS, TX--(Marketwired - May 11, 2016) - New research from Parks Associates shows only 4% of U.S. patients with three or more chronic conditions have a care coordinator and only 35% have two or more care providers. Most patients rely on their primary care doctor for routine exams, which creates significant potential for care providers to expand their roles and improve the patient experience through connected healthcare solutions.

"More than 30% of consumers in U.S. broadband households own and use at least one connected health or wellness device, and about 40-45% of consumers with chronic conditions have used at least one health app," said Harry Wang, Senior Director of Research, Parks Associates.
