Prevea Health and Phytel Announce Care Management Conference for Patient Centered Medical Homes

IBM Health Leader Paul Grundy, MD and TransforMED CEO Bruce Bagley Keynote Conference Focusing on How to Better Coordinate Care for Value-Based Success

GREEN BAY, WI and DALLAS, TX--(Marketwired - Aug 14, 2013) -  Prevea Health and Phytel today introduced the PCMH Care Management Conference, the premier industry event designed specifically to deliver critical intelligence, resources, and best practices for care managers and professionals involved in care coordination within patient centered medical homes (PCMH). The conference will be held August 28, 2013, at Lambeau Field Atrium in Green Bay, Wis.

The conference will host 15 distinguished panelists and speakers, including nationally recognized experts in care management, physicians and policymakers. The event will feature keynotes from Paul Grundy, MD, MPH, the founding President of the Patient Centered Primary Care Collaborative and IBM's Director of IBM Global Healthcare Transformation, as well as Bruce Bagley, MD, FAAFP, Interim President and CEO of TransforMED, a nonprofit subsidiary of the American Academy of Family Physicians.
