Self-Insured Schools of California (SISC) Partners With American Health Care ("AHC") on Diabetes Management Program

VISALIA, CA--(Marketwire - Sep 1, 2011) - Employees of 44 school districts in Tulare and Kings Counties now have access to a diabetes management program proven to significantly improve health and save the employer and employee money. The program is offered through a partnership between American Health Care, SISC and Buckman-Mitchell Insurance.

Using American Health Care's improved-Asheville care management model, school employees who choose to enroll visit monthly with a certified and trained Care Manager (e.g., a pharmacist, nurse, nurse practitioner or certified diabetes educator). Care Managers, in collaboration with primary care providers, proactively manage the patient's condition through assuring that they take their medications, have appropriate lab tests performed and through managing progress towards life style and diet goals. A key component of the program surrounds American Health Care's DOCS™ technology, which allows the Care Manager to easily identify and manage gaps in the patient's care versus evidence-based national guidelines. School employees can select a site for their one-on-one consultations, choosing from pharmacies and clinics in Tulare, Kings and Kern counties. Employees who choose to enroll also benefit from waived co-payments and pharmacy deductibles for certain diabetes, cholesterol and blood pressure medications as well as diabetic supplies so long as they remain compliant with program guidelines. Published results demonstrate that the program minimizes patient complications, reduces hospital visits, improves patient health and significantly reduces employer health care costs.
