South Country Health Alliance Chooses Sandlot Solutions for Proven Technology, Analytics and Quality Improvements

HIT Will Support the County-Based Health Plan in Its Transition to Value-Based Healthcare in Minnesota

FORT WORTH, TX and OWATONNA, MN--(Marketwired - Jan 28, 2014) - Sandlot Solutions expanded its reach in the health plan market today. The company announced that it was selected by South Country Health Alliance (SCHA), a county-based health plan serving 12 counties in Minnesota, to support its transition to value-based healthcare. SCHA will leverage the use of Sandlot's Connect, Dimensions, and Metrix products and technologies to support the new pay-for-performance contracts. Sandlot Solutions, certified by the State of Minnesota, is one of only four organizations authorized to provide health information exchange services in the state and facilitates data sharing between providers helping to decrease fragmentation and duplication of healthcare services.
