T-System Introduces New Care Coordination Innovations at the Emergency Nurses Association Leadership Conference

New Solutions Improve Connectivity Prior to Arrival and at Discharge to Enhance ED Performance

  • Hand-off connectivity: T-System will demonstrate a new solution focused on improving patient care continuity and supporting the ACO model of healthcare delivery. T-System Performance Care Continuity is an integrated, web-based referral management solution that strengthens hospital-physician alignment, helps reduce readmissions, and connects the care team to a patient's information. Attendees will see how automated workflows ensure that the patient-handoff process is seamless and smooth.

  • Pharmaceutical connectivity: T-System will present its new T-System Performance eRx solution, which gives clinicians the ability to create legible prescriptions and send them directly and securely to the patient's preferred pharmacy before the patient even leaves the ED. Attendees will learn how e-prescribing improves patient safety and satisfaction as well as the hospital's bottom line.

  • Performance measurement: Attendees will see how an intelligent console display of real-time ED performance data can help EDs better plan for staffing, identify bottlenecks and perform predictive analysis of their results.
