Telecare Wins San Diego Mental Health Director's 2012 Program of the Year Award

ALAMEDA, CA--(Marketwire - May 31, 2012) - San Diego County Behavioral Health Services Director, Alfredo Aguirre, LCSW, announced today that Telecare's mental health recovery program has received the San Diego Mental Health Director's 2012 Program of the Year Award. The program includes four unique outreach services including Telecare Gateway to Recovery ACT; Transition Team; Project 25; and Pathway to Recovery ACT and In-Reach Services, which provide an array of supports to individuals with serious mental illness and complex needs. The Award is presented annually to a program that demonstrates excellence, innovation, collaboration and advocacy in the delivery of mental health services.

"Telecare's program has been a model for innovative approaches to empowering clients with complex needs and for outstanding collaboration with other community mental health providers. In 2011, Telecare's program served over 1,000 adults and "transitional-age youth" (age 18-25) who have been diagnosed with a serious mental illness and who also experience other related psychosocial issues, such as: homelessness, chronic substance abuse, high use of Medi-Cal or emergency services, or treatment in locked institutional care settings. Its outreach and supportive programs (e.g. assertive community treatment, case management, and transitional care) are designed to coordinate closely with families, hospitals, providers and other community mental health care organizations in San Diego County.

The award will be presented during at the 26th Annual Mental Health Recognition Dinner, June 1, 2012, at the San Diego Marriott Hotel Mission Valley.
