uniPHARM Wholesale Drugs Proud to Announce Inclusion of 19 Stores Into the B.C. Medication Management Project


VANCOUVER, BRITISH COLUMBIA--(Marketwire - Jan. 27, 2011) - uniPHARM Wholesale Drugs is proud to announce that 19 of its stores have been selected by the government of British Columbia to participate in a province-wide government pilot project called the B.C. Medication Management Project.

The B.C. Medication Management Project is aimed at engaging B.C.'s pharmacists to assist in expanded patient care. The project sets out to provide all British Columbians with systematic patient consultation, medication review, a customized care plan, and follow-up care. 

Medication Management is a comprehensive service where pharmacists work together with patients to optimize medication use. The service includes preparing and reviewing a detailed medication history, identifying any medication management issues and setting patient-focused goals. By implementing solutions to address issues and meet goals, and by monitoring and modifying solutions according to changing patient needs, pharmacists strive to prevent further medication management issues. In addition, pharmacists document care and communicate/collaborate with other members of the patient's health care team, offering a multidisciplinary and patient-centred approach to patient care.

For uniPHARM's roster of community pharmacists, Medication Management is not a new concept, rather, a way to systemize the one-on-one patient consultation and specialized services that have made them longtime staples in their respective communities.
