University Hospitals and Emmi Solutions Partner on Commitment to Patient Engagement

Record of Success Garners a Long-Term Agreement

CHICAGO, IL--(Marketwired - Feb 4, 2014) - University Hospitals (UH) in Cleveland, OH has entered into a long-term strategic relationship with Emmi Solutions, a leader in outcomes-driven patient engagement. UH and Emmi Solutions are committed to working together to create and implement programs to increase population health outreach, enhance the transitions of care process, assess patient satisfaction and improve the health and wellness of patients.

The decision to enter into a long-term partnership is founded on UH's leadership in population health and Emmi Solutions' innovations in engaging patients. University Hospitals' ACOs now include more than 170,000 self-insured, commercially insured, Medicare and Medicaid members and beneficiaries, making it one of the largest ACO initiatives in the country and one of the few providing coordinated care, disease management and case management services to all populations, from infants to senior citizens.

UH and Emmi Solutions have been working together to improve patient outcomes since 2010 with a number of successes, including a combined effort that led to a 13% improvement of a key HEDIS measure for colorectal cancer screenings. Emmi Solutions' multimedia programs are delivered through a platform that creates interactive, engaging encounters to motivate patients to take actions essential to improving their outcomes and care while also tracking patient feedback to improve current processes.   • EmmiTransition - Interactive phone calls and multimedia programs support clinical staff to effectively guide patients and their circle of care though the care transition process. Each call asks self-reporting questions about patient condition and compliance and delivers responses to providers so they may more efficiently leverage their staff. The multimedia programs deliver easy-to-understand, actionable messages designed to help keep patients on the right course.
  • Appointment Reminders - Through interactive voice response (IVR) technology, appointment reminders contact patients to confirm, reschedule or cancel scheduled appointments. Using an empathetic voice and language patients understand, reminders provide important educational information specific to patient appointments and reinforce key messages.
